Sick Red-Eared Slider

Hi everyone,
What would cause a R.E.S. to scratch his side constently to start a red spot with a tiny bit of blood on both sides of his body??
Its a 6 ft. tank, it has 6 inchs of water at the deepest so probably 25 gals of water. Filtered through a fluval 204, ramp out of water onto a basking spot. He is fed crickets as a treat a turtle food as staple. The only thing I can guess is maybe the basking spot is too hot, and he wont move away, so its drying his skin out. Anyone else encountered a problem like this??
