Reptile Educational Show Permission Forms????

Hi All,
I have started doing some small educational shows using some of my repitles and inverts. I am doing a rather large one in the near future and the smaller ones are becomming more frequent. Just wondering what you all thought about the idea of some sort of "Permission" form to either have the teacher, pricipal, or each of the students parents sign.
I think someone may have posted a good one as a PDF a while back cause I remember looking at it when it was first posted but have been unable to find it again in my searches.
I just want to cover my butt for these shows "just in case". So far I have had a sort of "hands off" policy when it cames to kids around the animals but I want to really cover my bases here.
