I saw the same sort of thing happen when I was an apprentice. One of the guys in the machine shop took the tip off his finger trying to clean swarf (metal shavings) from the tool on his machine. After two weeks he returned and after getting an earful from the boss for his stupidity he came back into the shop and started working on the same machine. One of the other guys asked him what had happened, he showed him and promptly took the tip off another finger. He was suspended for a month. :-)
